The webpage contains most information you will have questions about. As this page is new with our new enrollment company, updates are being made regularly. However, plan information, rates, claim forms, contact information, 2018-19 Benefit Guide, etc. should be available on this site.
New Benefit ID Cards
(BE SURE YOUR ADDRESS IS UPDATED IN EMPLOYEE ONLINE SO YOUR CARD IS DELIVERED APPROPRIATELY.)
BCBS will issue new ID cards for anyone that changed plans (EPO to HDHP or HDHP to EPO) and all EPO members to reflect new copay amounts. HDHP current members will not receive new cards. Right now, cards are targeted to be mailed to be received by the member June 22 through June 29. If you don’t have your new card by July 1, please call BCBS’s Customer Service (telephone number below). If you are new to the HDHP and will have a new HSA (Health Savings Account), you will receive a Welcome Kit directly from UMB. This will also not arrive until approximately June 22-29. If you don’t have your Welcome Kit by July 1, please call UMB’s customer service telephone number below.
Delta Dental will only issue new ID cards to new members. Current members will not receive a new card. If only a dependent was added to a current member, a new card will also not be issued. To request a new card, if needed, please see Delta Dental’s contact information below.
EyeMed will send a welcome packet to all new enrollees. Current members will not receive a new ID card. If you need a new card, please see EyeMed’s contact information below.
Guardian (Voluntary Life, Voluntary AD&D, Critical Illness, Accident) will not send any information to members. You may log on to www.guardiananytime.com to confirm your coverage after July 1.
Surency will send new debit cards to all FSA (medical and/or dependent care) participants. These should arrive close to the last week of June. As of July 1, your current Benny Card will no longer be active.
Retiring Employees: You will receive information from Surency Advantage Plus with information about continuing your benefits either as a retiree or through COBRA. This information may not be available until approximately 2-4 weeks before your benefits end with NKCSD. Surency contracts with a third party vendor for assistance with payment collection call the TABEN Company. Please be watching for information from either Surency or Taben. You can also reach Surency directly at 855-889-7438 with questions.

Meritas Health Clinics are still available during the summer to EPO plan members with no office visit copay. Please see the linked flier for locations and times.

Forms & Flyers
Changes on the EPO Plan
- Primary care physician copay will increase from $35 to $40
- Specialist copay will increase from $70 to $80
- Urgent care copay will increase from $70 to $80
- Chiropractic Office Visit and Skeletal Manipulation copays will increase from $70 to $80
- High Deductible Plan deductible will move from $1,300 to $1,350 per IRS Regulations
- Both plans will utilize Rx Mail Order Incentive – Information was in the May 10 e-Insider and linked here.
- Both plans will utilize Rx Mandatory Generics – If you are affected by this change, a letter from BCBS will be mailed to you directly to your address of record with BCBS
Benefit vendors can answer questions about claims, whether a service is covered, assist with lost ID cards or new ID card that haven't been received, find an in-network provider, etc. They cannot assist with address changes (must be done through Employee Online), enrollment changes (need to go through Benefits Specialist), etc.
