Forms & Physicals
Prior to participating in school athletic activities, students are required to have a current physical on file with their school per MSHSAA guidelines.
Pre-Participation Sports Physical Evaluation History Form
Following a concussion, the Return to Play Form must be completed and on file before an injured student can return to their activities. See other information regarding heat, hydration and conditioning recommendations as well as other MSHSAA resources on the MSHSAA website.
Please note that per Missouri HB-300 regulations, MSHSAA has released its annual Interscholastic Youth Sports Brain Injury Report regarding the impact of student athlete concussions and head injuries and the efforts that have been made to minimize damages from school sports injuries on its website. This report is located by going to www.mshsaa.org and then going to the Sports Medicine tab.
