    **NEW INFORMATION REGARDING SPORTS PHSICAL REQUIREMENTS**

    All sports physicals will be collected and stored through PRIVIT.  Below are instructions for parents to learn how to create and manage an account.

     

    MSHSAA Sports Physical Form - this is the form your doctor will need to complete before you upload to PRIVIT

    PRIVIT Link - MUST be completed and updated anuually

    PRIVIT - Parent Instructions for New Mark Middle School

    • Parents of 7th grade students, or new students to New Mark, will create/register their student in PRIVIT during their 7th grade year and will update Annual Requirements during the 8th grade year of school 
    • Parents of 8th grade students, who did not participate in athletics previously, MUST create/register their student in PRIVIT following the instructions above
    • Parents of returning 8th grade athletes MUST complete Annual Requirements in PRIVIT during their 8th grade year
